Does anyone know how this form will effect the 90 day reporting by post? Or indeed that reporting by post is still an option.

The form clearly has a space for the interviewer/recording person to sign. On that basis it would appear that you have to be present.

I have already done a report by mail since all the topics about these forms started and had no form to send with it.

How could they expect you to submit one if they have not given you one to use. Their is no official download of one available.

Just mail in your report without the form.

And don't forget that you'll need to go to your local immigration office in person sooner or later to extend your stay by a further year. They'll probably ask you to complete the form then.
