There is no law covering lost/replacement 90 day report slips or TM6 cards, or any associated fines for replacements.

 

It's your responsibility to keep your passport, 90 day report receipts and your TM6 cards securely, not Immigrations.

Not having the previous slip can create problems if immigration does not have a record of it in their computer.

The office here misplaced mine when doing my 90 day report and extension application at the same time. My record of reporting had also disappeared from their computer. They wanted to fine me 2000 baht but I ended that when I asked about finding the original TM47 for my previous report. They went to a cabinet and dug out 3 days of reports  since I was not sure what day I did the report. Each stack of reports had a printout of the reports done that day and sure enough my name appeared on one of them and I found my report. They then did my new report. The next year when I did a report and extension at the same time I stapled the slip to the TM47 form so it would not get misplaced again.

Phuket can do cause extension desk and 90days desk are just near eachother but Bkk cannot cause different area and queue has different letter and numbering ticket

Do you understand the role of a moderator? Clearly not! This is a forum where all members post information, opinion and advice and a moderators opinion is no more relevant than any other member.

 

ubonjoe quoted the law on penalties for failing to report which is not the question being asked by the OP. I was simply clarifying that there is no law/penalty concerning the OP's question.

Found an advice yesterday at immigration:

"In case of lost 90days report, the person must go to local police and make lost declaration and then get back to immigration with that document"
