Whether you’re already in Thailand or someone who’s considering relocating to Thailand, healthcare is an important aspect to consider. As the world recovers from the impact of COVID-19, this might be a good time to re-evaluate your healthcare plan. According to the latest Cost of International Health Insurance Report 2023 by Pacific Prime, the average premiums for international private medical insurance (IPMI) plans are increasing across the globe. Thailand - still one of the cheapest countries to get insured When looking for a health insurance plan before you look at the nitty gritty details, you should consider your budget and the cost of health insurance in Thailand. Based on the data collected, in Thailand, the average premium for individual IPMI plans in 2022 stood at USD $3,594, a whopping 77% increase (one of the highest from the data collected) when compared to the previous year. Despite the massive increase, Thailand is still one of the cheapest countries to get insured in–the country ranked 71 for the most expensive countries to get insured in (out of 100 countries). Looking at family plans, Thailand also saw a steep surge in cost. In 2021, the country ranked last for the most expensive country to get insured based on the average cost of international health insurance for families among the 100 countries surveyed. In 2022, the average cost stood at USD $11,683, and the country rose to the 50th spot with a significant 59% increase when compared to 2021 (USD $7,367). Possible causes of cost spike The cost spike had been caused by a number of factors, from the general life expectancy increase to just good ol’ fashion inflation. As life expectancy increases, the need for medical services will also increase. In 2019, the Thai government also extended their dual pricing policy to include public hospitals, directly raising the price of medical services for expats who are classified as tier 2 on the pricing system and retirees and tourists who are classified as tier 3 (most expensive). This could directly affect the cost of health insurance in Thailand for foreigners. After COVID-19, the general public have become more aware of their personal health and maintenance. Although there’s no specific treatment for PCC yet, a research published in the peer-reviewed Internal Journal of Molecular Sciences suggests that cannabis might help manage these symptoms due to its anti-inflammatory and antioxidant properties. Understanding long Covid Post-Covid condition or Long Covid refers to the persistent symptoms following a Covid infection. These symptoms can last for weeks or even months after the initial illness. The World Health Organization (WHO) classifies PCC under ICD-10 code U09.9, reflecting the significant health challenge it poses. PCC can affect various body systems, including respiratory, cardiovascular, and neurological systems. Neuroinflammation, characterised by inflammation in the brain, is a major component of PCC, causing various neuropsychiatric symptoms. Cannabis compounds and their properties Cannabis contains several compounds, known as phytocannabinoids, which have shown promise in treating various health conditions. The most well-known of these compounds are cannabidiol (CBD), cannabigerol (CBG), and Delta-9-tetrahydrocannabinol (THC). These compounds have significant antioxidant and anti-inflammatory properties, making them potential candidates for treating neuroinflammation in PCC. CBD: A potent antioxidant and anti-inflammatory agent CBD is a major component of cannabis with numerous health benefits. It interacts with the body’s endocannabinoid system, which regulates various physiological processes. CBD has been shown to reduce inflammation and oxidative stress, two key factors in PCC. Studies in animal models have demonstrated that CBD can significantly reduce levels of proinflammatory cytokines, which are molecules that promote inflammation. For example, a combination of CBD and THC has been shown to reduce inflammation more effectively than either compound alone. This synergy could be particularly beneficial in treating the complex symptoms of PCC. That’s the surprising and alarming reality for some users of kratom, a plant-based supplement gaining popularity for its opioid-like effects. Widely available in convenience stores, smoke shops, and online, kratom is now being linked to cases of hyperpigmentation, most notably on the face and hands. What is kratom? Kratom, a plant native to Southeast Asia, is commonly used for its analgesic properties. Often marketed as a ‘legal opioid’ or ‘legal high,’ it can be found in small containers of extract or as a powder. It was decriminalised in Thailand in 2021. People consume it in various ways: boiling the leaves into tea, smoking, chewing, or taking it in capsule form. Despite its widespread use, kratom is not regulated in the United States, leading many to believe it’s a safe herbal supplement. However, kratom has several side effects and can be addictive. Side effects and addiction risks Many users are unaware that kratom can cause drowsiness, rapid heartbeat, vomiting, respiratory issues, cardiac arrest, confusion, and hallucinations. The plant also affects the psyche differently depending on the dosage. At low doses, it acts as a stimulant, while at high doses, it has a calming, pain-relieving effect. Long-term high-dose users are at risk of developing hyperpigmentation, a condition where the skin darkens in sun-exposed areas. One case involved a 54-year-old man who used kratom to reduce opioid use. Over four to five years, he developed diffuse hyperpigmented patches on his arms and face, sparing the knuckles. Another case detailed a 30-year-old man with no other medical conditions who presented with dark grey-blue skin on his cheeks, neck, and hands. This man started using kratom after becoming addicted to opioids following a high school injury. Despite stopping kratom use in May 2022, his skin discolouration remained for at least 16 months, raising concerns about whether the hyperpigmentation can ever fully regress. Doctors in Thailand have also recorded cases of skin hyperpigmentation among users of Kratom. The mystery behind kratom’s skin effects The exact mechanism by which kratom causes skin discolouration is still unclear. One theory suggests that increased dopamine levels from kratom use may stimulate melanocyte activity, leading to hyperpigmentation. Another possibility is that a melanin-drug complex forms and deposits in the skin. The discolouration typically appears in areas exposed to the sun, such as the face, neck, hands, and arms. Potential treatments and precautions Currently, the best treatment for kratom-induced hyperpigmentation is to stop using the supplement. Some success has been seen with Q-switched laser treatments, which are used to treat other types of dark skin discolouration. However, it’s still uncertain how effective this method is for kratom-induced cases. There’s something about those white South African guys that seems to make them much thicker-skinned than the rest of us. Perhaps it’s the fact that they live in a much more violent country than most, with South Africa’s crime rate being one of the highest in the world. Or maybe it’s because so many of them seem to work as mercenaries across the continent, taking on dangerous and high-stakes jobs that would make most people quiver in their shiny Timberlands! It’s known that much of the white-dominated armed forces of South Africa were left with little to do after the collapse of Apartheid, which resulted in a huge increase in private military recruitment. Given the volume of wars occurring across Africa in the 90’s, there was a real need for such highly trained military personnel, with companies like Executive Outcomes forming to supply the manpower. Whatever the reason, these guys are known for their toughness and resilience, and one such individual who in recent years gained international attention is Leo Prinsloo. His reputation proceeds him. If you’ve been asked to personally protect King Charles III, the Kardashians, and Nelson Mandela, you’re clearly no shrinking violet. Prinsloo is a former member of the South African Police Service’s elite Special Task Force, a unit that deals with high-risk operations such as hostage situations, counter-terrorism, and violent crime. The 52-year-old is also a tactical firearms instructor and the founder of the Edge Shooting Academy, which trains individuals in advanced shooting techniques. But it’s not his impressive resume that has people talking – it’s his incredible display of bravery and skill during an attempted heist that has earned him the title of “badass MF”! Attempted heist and Prinsloo’s heroics On April 22, 2021, Prinsloo was driving an armoured cash-in-transit Toyota Land Cruiser in Pretoria, South Africa, when he and his partner, Lloyd Mtombeni, were ambushed by a group of armed criminals. The attackers opened fire on the vehicle, attempting to force Prinsloo and Mtombeni to stop and hand over the mere US$100,000 worth of mobile phones they were transporting. But Prinsloo had other plans. As the AK47 bullets popped, Prinsloo remained calm and collected (despite his heart probably beating at 200 bps!), expertly manoeuvring the armoured vehicle to evade the attackers. He can be heard in the dashcam footage telling his partner to “phone Robbie, phone Josh” – presumably to call for backup – while continuing to drive and evade the criminals. At one point, Prinsloo even manages to ram one of the attacker’s vehicles off the road while maintaining his composure and focus. Eventually, the attackers realized they were no match for Prinsloo’s skill and determination and decided to flee the scene. Thanks to Prinsloo’s quick thinking and bravery, he and his partner were able to escape unharmed, and the cache of phones they were transporting remained safe. Public reaction and praise for Prinsloo Once the dashcam footage of the attempted heist went viral (more than 60 million views across a few videos), people worldwide were quick to praise Prinsloo for his incredible display of courage and skill. Many have called him a hero, while others have dubbed him a “badass MF” – a well-deserved title, considering the dangerous situation he faced and the calm, collected manner in which he handled it. Prinsloo’s actions have also shone a light on the dangers faced by those working in the cash-in-transit industry in South Africa. According to the Institute for Security Studies, cash-in-transit heists have been on the rise in recent years, with criminals becoming increasingly brazen and violent in their attempts to steal large sums of money. That, alongside their ability to bribe the security forces and local police. Prinsloo’s heroics serve as a reminder of the bravery and skill required by those who work in this high-risk field, and the importance of proper training and preparedness in the face of danger. Of course, the keyboard warriors (likely the same ones that would cry for pointing out Prinsloo’s race) were quick to put down Prinsloo’s partner, Lloyd Mtombeni – as if they’d have reacted better in such a situation. Mtombeni can be seen in a state of shock when it all kicks off, but he doesn’t panic and appears to remain incredibly calm, the exact behaviour required by Prinsloo, with whom Lloyd’s life was firmly in the hands of. How would you have reacted listening to the pops of AK47 bullets going off around you and you’re the target? What can we learn from Prinsloo? Prinsloo’s incredible display of courage, skill, and composure during the attempted heist is a testament to his training and experience as an elite Special Task Force member and a tactical firearms instructor. His actions serve as a powerful reminder of the importance of remaining calm and focused in high-pressure situations, and the value of proper training and preparedness. For those of us who may not face life-threatening situations ever in our lives, there are still valuable lessons to be learned from Prinsloo’s actions. His ability to stay calm under pressure, think quickly, and make decisive actions in the face of danger are qualities that can be applied to various aspects of our lives, whether it’s dealing with a difficult work situation or navigating a personal crisis. And, when you think you’ve had a bad day, just remember how much worse April 22, 2021, was for Prinsloo and Lloyd. Let’s not forget to mention the car/weapon that seriously contributed to the pair’s survival. That armoured Toyota Land Cruiser survived a dozen or so shots from close range, not to mention the other vehicles ramming it. So the next time you’re on a road trip through Afghanistan or Iraq, you know what car to rent! Is Prinsloo the bravest person you know of? It’s certainly possible, especially as Batman isn’t real! His incredible display of courage and skill during the attempted heist has earned him international praise and admiration, and his actions serve as a powerful reminder of the importance of remaining calm and focused in high-pressure situations. While most of us will never come close to the level of danger Prinsloo faced, his story can still inspire us to cultivate our resilience, determination, and ability to stay composed under pressure, whilst keeping everything in perspective.
