Because you are arriving with a valid re-entry permit, not with a valid visa, and because data entry for the field "Visa No." is not compulsory, you are free to leave that field empty. That's what I would do in your situation.

Your "12 month retirement extension" is an extension of your temporary permission to stay in Thailand and for immigration purposes your country of residence is not Thailand. However, it is unlikely that indicating Thailand in that field will get you into any trouble.

For "Country of residence", the trick is to type the first three letters of the country name, eg gre for Great Britain produces a pop-up with a few county names that includes Great Britain; uni for United Kingdom gives a list that is too short to include that name, but continue trying and unit brings up the list that includes United Kingdom. Of course, if I happen to know the ISO three-letter country code, eg SWI for Switzerland or GBR for Great Britain, this is the most direct way to fill in the field for the country of residence. (Lower-case letters are also accepted for the codes)

I noticed the same on my phone. The trick is not to try to scroll, but to type the first three letters of the nationality, eg swi for Swiss, bri for British.

I have not yet seen an official list of required documents for an extension of stay based on a DTV. Could you post a copy of the list you received from the Jomtien immigration office, please?
