I think you try to do two steps in one. The screenshot you've posted is for the second step, ie E-visa application. Before that you need to obtain the letter of approval from BOI LTR. That's when u need your pensions statement and health insurance certificate. You can make your application to BOI here. To get this letter will take about a month.
