Police aren't in the business of chasing hospital bills. It's not their duty.
At the moment she owes the hospital money, not him.
She can sue him sure but it won't get that far. She couldn't afford the legal bills I'm afraid.

In the end she owes the hospital money and banking in a promise to have it paid.
He might not have even promised to pay. Could be her just saying that to leave the hospital.

Hope they catch him so I can hear more details.

Drop the lecturing on stuff about you can only talk in therory. This is Thailand, Police normally acts as mediator and immediate remedy in many case. Don't like that, leave or at least stop repeating your nonsense.
 
Face reality.
Police are only interested in the overstay.
He hasn't broke the law otherwise.
