At Suvarnabhumi, there is a veterinary office which can issue health certificates needed for pets (like cats, dogs) going on international flights. 
Is there anything of the kind at Phuket Airport?
I would like to come to Thailand with a dog. Can I book a flight straight to Phuket airport?

Your post is rather unclear as to whether you want to import or export a dog. Two totally different things! Yes Phuket Airport has an Animal Quarantine Office. It is in the cargo building past the main domestic entrance and exit on the left. It's on the second floor, about half way down the row of offices. If you are exporting a dog you must meet the requirements of the country you are taking your dog to. Different countries have different requirements. Thailand requires at the minimum, current rabies vaccine and the parvo virus combination vaccine along with a micro chip in your pet. You must bring your dog to the office at least 10 days before departure but not more than 12 days. They scan your dog's microchip. Also copies of your vaccine records as well as originals. A copy of your airline ticket and passport ID page. You have to go back and pick up the export permit 3 days before your departure, which is a total pain! I had their phone number, but can't find it now, call them to make sure they are open and exactly what they want as the rules change. Importing a dog is a whole different process.

Yes there is an Animal Quarantine Office at Phuket Airport. Bringing a dog to Thailand you must get a health certificate in your home country and stamped by your government. It is then checked by the airport Animal Quarantine Office, they don't check your dog. There are several breeds that are not allowed to enter Thailand. There are also several breeds that airlines don't allow to fly in the cargo hold. Check your country's Thai Embassy website, it should have a list of requirements.
