Deblock Jap I Phone

Dear All,

 

Need to deblock my japanese IPhone5. Saw somebody having just added something at the back of his SIM. 

 

Which place in either Bangkok or Chiang Mai would you recommend to get it? How much would it be? Heard MBK is very expensive, somebody told me 800 Baht which is very high indeed.
