Extensions of stay after using Re-entry permit for add’l year

I get a new O-A visa in USA every other year , carefully timing visits so that I get an extra year by getting a re-entry permit just before the visa expires. I have always thought that I must get a new O-A when the re-entry permit date expires and am not eligible to apply for an extension of stay in Thailand. Is this correct ?

In reality you re-enter the country to get a further year, just prior to the visa expiring. Then a Re-Entry Permit from Immigration to maintain that permission to stay date in case of leaving Thailand again. 

There is no reason you could not apply in Kingdom to get an one year Extension of the permission to stay, It would require proof of money in a Thai bank or income from overseas into a Thai bank.

No. You can apply for a 1 year extension of stay within the last 30 days of any of the 1 year stays issued when entering the country. Any valid re-entry permit is irrelevant.
