Under Thai law lastnames are family specific, you cannot have the same lastname as another family. 

 

The problem is probably that they don't understand the concept of a middle name. They probably see it together with the real lastname a double last name.

    If it's a foreign grandmother, just make sure the middle name is spelled correctly in Roman script on the foreign passport, let them spell it how they like in Thai script.

    My daughter was born in Bangkok and we registered her with 2 middle names without any problem. I think you need to stand your ground and insist on it.
