To apply for the yellow vaccination passport in my province you need to provide your passport, your double vaccination certificate and pay 50 Baht. Quick update, Pattayrak now accept walkins (won't take a booking) but if you're not registered on Mor Prom then it's probably a waste of time going there, or at least it was for me today where I was told I needed to go back to the hospital in Sri Racha who did my vaccinations to get them to update my records ????
