You only need 30 days of covid 19 insurance. That is has already been discussed many topics.

Seen some rumblings, source: Tourism and Sports Minister Phiphat Ratchakitprakarn, that insurance may be dropped, especially after the 300 baht increase in the passenger service charge (increased from 700 baht to 1,000 baht) kicks in.

There is no covid insurance for O-A extension.

It will not be required until after September of this year and will be included in the new insurance required after that day. See: https://longstay.tgia.org/guidelineoa

If you got your OA visa after October 1st of 2021 you will would need it to enter the country.
