Is everything open and running as normal between Koh Lipe and Langkawi? Is this a feasible way to get a new visa exempt to Thailand?

The ferry transport between Lipe and Langkawi is not operational.

You have to go through Satun at least.

Are you sure this is still true? I've seen reports saying the planned reopening is early November and a ferry website that is currently selling tickets.

Obviously, last month was before early November, and your well intentioned warning is probably out of date.

Yes I hope its on again down there it should be, Malaysia has dropped its COVID requirements for entry according to IATA website COVID travel information map. Ko Lipe is a great place to snorkel and I'm planning to go down there in February or so when there is good visibility. I would think its a great place to do a visa run too.
