So I know that coming on the internet for medical advice is not clever,

You need to see a highly experienced and referred ENT.  I suggest asking @Sheryl for a recommendation for the best ENT in Chiang Mai (in her opinion).  She often recommends a doctor in Bangkok especially for surgery.
If I was having the issues you were having I'd probably book a flight to Bangkok and visit that particular ENT that Sheryl recommends.

The doctor wanted you to get a CT scan probably of your sinuses.  That is also probably what you need as part of the investigation to your issue.  This way  the doctor can look for any abnormalities other than using the endoscopy.
Unfortunately you don’t have the money to get one .  And it sounds like you don’t have insurance.

They do work is all I can say. Actually my nose got broken one time and I thought about having an op. done as my left nostril that was completely closed, after using these it just cleared, now sometimes either nostril will sometimes block but these little beauties do the trick in 5min I feel the congestion come into my mouth and then just spit it out. nice talk at lunch time init. I also needed oxygen sometimes before sleep now no longer needed. 👍🏻

I was experiencing headaches when in U.K. My GP referred me to an ENT specialist. This was before our hospital had CAT/MRI scanners. X-Rays were taken and the specialist informed me; 'Your sinuses are all aerated'. He could do no more. Eventually, I was advised that the cause was an unnecessary prism incorporated into my last pair of spectacles. What I am pointing out is that a scanner is not necessary to investigate your sinuses. X-Ray's can do the job too.

I sympathize with you, having had similar problems all my life.  For a long time 10 mg Claritin tablets (non-drowsy antihistamine and dirt cheap generic loratadine tablets are sold in Thailand) and Flixonase (fluticasone propionate) nasal spray worked for years.  Tramadol HCL 50 mg tablets also work well as an antihistamine and are also sold as a generic in Thailand.

Eventually Claritin and Flixonase were no longer enough so I opted for nasal surgery at Ramkamhang Hospital in Bangkok to enlarge the entrances to my sinuses.  Having the surgical packings removed was incredibly, unbelievably, painful.  It the worst surgical experience of my life.  Looking back I should have simply had my deviated septum fixed and seen how that worked before agreeing to further surgery.  I will regret having the full surgery to my dying day.

The good thing that while I still have some problems I no longer have pain in the sinus above my eye.  I still take loratadine or tramadol HCL when necessary.  If things get really bad I start using Flixonase again for a few weeks.  If I start having clogging problems because of thick nasal secretions I add Acetylcysteine (which thins phlegm) 2 or 3 times a day - it is available in 200 mg soluble power form in sachets at most pharmacies.

I avoid the use of decongestant nasal sprays as much as possible but if absolutely necessary I will use one or two sprays of a generic 0.05% Oxymetazoline spray once a day, which is usually at bed time.  Warning: decongestant nasal sprays can have a rebound effect if used too often so even with a clogged nose from a cold use it only two times a day, one nostril in the morning and the other at night.

If you want to do a nasal lavage you can buy bottle of normal saline in most pharmacies or simply dissolve 1/2 teaspoon (about 3.5 gm) of kosher salt in 1 cup (235ml) of bottled water to produce a much less expensive normal saline solution.  I use a slightly higher concentration of salt, about 5 g per 235 ml of bottled water.

You can use regular iodized salt but kosher salt doesn't have any iodine or anti-caking additives and I find it gentler to my nasal tissues.  The rock salt sold here in Thailand for salt encrusting fish before cooking works fine.  If you don't like the large crystals you can grind some up in a mortar and pestle to produce small salt grains but the small grains will probably cake since they don't have additives.  Warm the saline solution to the same temperature that you would warm a bottle of milk for a baby (98.6ºF or 37ºC) and use either a large syringe (obviously without a needle) or use a large rubber bulb ear syringe for the lavage.

Just as an added note, iodized salt adds a slightly bitter aftertaste to food so food cooked with the rock salt tastes better.  Sea salt, on the other hand, adds a taste that can very from source to source and you use it and which you use is a matter of whether or not you think it adds a good taste or not.

I have a deviated septum which was discovered with a Naso endoscopy procedure. I don't notice much blockage on the one side most of the time, but just the slightest allergy or laying down at nights will totally block that one side. I use one spray of Oxymetazoline HCL .05% solution in each nostril before bed, and it will keep my passages open for a full 12 hours. The best and least expensive spray I have found at nearly all pharmacies is Phindroz. I pay 70 Baht for a 10ml spray bottle. Good stuff!

Same thing happens to me  ,do your eyes ache?   Any sores inside your mouth?    My Dentist in the States said it was a sinus infection. My eye doctor checked me for Glaucoma    .  How is your blood pressure?  Do you look at computer screen for hours?  If you find out what it is let us know. The Eucalyptus stuff does seem to work pretty well over here.

Hi, you may need a minor op to open the Sinus passages, I had trouble for years with sinus infections due to the sinus canals beeing too small. First you need to get a CT scan done of your sinuses. Dont know anybody here in Thailand that does the sort of op, and to be quite frank I would not have a surgen touch me here. You need a very professional specialist to conduct sinus surgery, hence I had mine done in home country of Australia.

I also had surgery on my sinus passages to open them up. Thankfully, the procedure really helped me breath better. Taking drugs everyday to help is miserable and expensive and only gives you temporary releaf.

This will really help you. Read it and then read it again.

It seems you've tried the antibiotics but you have not really tried antibiotics which are ideal for sinus inflammation. So try those. See the link.

Simultaneously, I would do the steam inhalation in case it is a virus infection.

If none of that shows relief, and you still want to take the cheap route, you have to think about a couple of weeks course of antifungal medicine in case it is a fungal infection.

I would also blast specialist saline solution up your nose. You can get this in 7-Eleven. Use 50 mls for each side of your nose.

I would be very reluctant with all the sprays since although they might work in the short term they often have their own side effects later on.

Sounds daft but what I do is dip my pinkie finger in Listerine then stick it up my nose and then block the opposite nostril and inhale/snort.

Logic? If Listerine kills bacteria in our mouth it probably also kills bacteria  in our sinus.

I then begin to look for alternate solutions, like inhalations and sprays. Upon reading this forum, people were advising against those 'Thai' sprays found in pharmacies and mentioned Flixonase or Beconase (which i use to take back in Belgium when i was younger). But apparently, you cannot find them in Thailand. Now, i don't use any sprays as i don't want to depend on them and create further complications.

I also saw some people mentioning Ephedra, which is a Chinese medicine, apparently.

You mention biofilms, but i'm not sure what are they?

Flonase = Fluticasone Propionate ( I get mine from the U.S )

Flixonase = Fluticasone Propionate ..  available ant pharmacy in Thailand

Avamy's =  Fluticasone Furoate   same same but FF and FP are completely different drugs with FF showing distinct and superior properties also available in any pharmacy in Thailand

also...  look into the possibility that you may have "Athletes's Foot" in the nose

Providers classify fungal sinus infections as invasive and noninvasive. Invasive infections can spread to other areas, such as the eyes and brain.

  • Allergic fungal rhinosinusitis: This is the most common type of fungal sinus infection. It results from an allergic reaction to fungi inside the nose. Untreated, the sinuses fill up with thick mucus and nasal polyps can form. People with asthma or allergic rhinitis (hay fever) are more likely to develop this condition.
  • Fungal ball (mycetoma): Fungi build up in the sinuses and form a clump or ball. The fungal ball can get bigger and block the sinuses. Providers use surgery to remove fungal balls and clean out the sinuses.

The issue is located on the right eye only and yes it ache, although not terribly bad. Some drops do the trick. But its a constant nuisance. I have no idea how to know how is my blood pressure honestly... but i do look at the computer for hours (because of my work), and it sure doesn't help.

What about your right ear?  What drops do the trick might try them?

Thank you for the very helpful post. This article is great. I don't have any idea about antifungal medicine tho. I'll have a look

Antibiotics are difficult to get to the sinus so I use this Neti pot to flush and clean my sinus. Available on Lazada or Shopee
