Construction Accident: Engineer Predicts Locking Failure, Fatal Formwork Collapse

In a construction incident near Rama 2 Road Soi 82, Samaedam Subdistrict, Bang Khun Thian District, Bangkok, engineers are anticipating a potential locking mechanism failure leading to a tragic accident. Preliminary reports indicate that a construction worker lost his life, and another sustained injuries when an iron formwork collapsed on them while working on an expressway bridge.

On December 15, at 5:22 PM, police officers from Samaedam Subdistrict, along with rescue teams, rushed to the scene. The inspection revealed one fatality, identified as Mr. Thien, a 35-year-old Myanmar national, pinned under a broken iron formwork in a lifeless state. Another injured worker, Mr. Kyaw, 44 years old and also from Myanmar, was trapped beneath the fallen structure. Volunteer rescuers promptly transported him to PMG Hospital.

According to the crane operator, Mr. Kumpang (surname withheld), as he was preparing to lift the iron structure, an unexpected loud noise preceded the collapse, reported Siam Rath.

 

Unfortunately, two Myanmar workers were beneath the structure at the time, and due to the noise, he couldn't warn them in time. The incident is believed to be caused by a malfunction in the locking mechanism during the relocation of the iron formwork, resulting in an imbalance and the subsequent tragedy.

Mr. Piseetdech Khaowong, the construction project control engineer, stated that a detailed investigation is underway to determine the root cause of the accident. The authorities are awaiting forensic and engineering analyses to provide a comprehensive understanding. The deceased worker will be sent to Siriraj Hospital for autopsy, and legal procedures will be initiated in coordination with the family to ensure their rights are upheld during the religious funeral rites.

These form have very likely been used dozens of times.....

Is that not where forms are for??

Not saying it happened in this case, but drugs or alcohol would be a reasonable thing to look for in case of an industrial accident.

I am sure those Police with the Kraut Rommel hats are fully qualified to conduct lifting operations incident investigations.

 

All fully qualified and well experienced in Kelvin TopSet or equivalent methodology to determine root cause analysis.
