I presume I wont get treatment under the Thai equivalent of the NHS

Not for free unless you are legally married to a Thai Government employee.

You can get cheaper rubbish insurance, keeps some people happy, ignorance is bliss.

Also use govt hospitals where possible and keep healthy (diet and exercise) and keep/save enough money to cover emergencies and the inevitable ops you'll need

Thai government hospitals are very inexpensive.

Not for catastrophic illnesses and injuries, they aren't. Bills can easily exceed 1 million baht in such cases if for example highly specialized surgeries and long ICU stays are required.

Still 1/3 - 1/5 cost of private hospitals, but far from cheap in such instances.

Depends on age and type of cover (hospitalization only vs also outpatient etc) and how much deductible ("excess").

Cigna is comparatively inexpensive relative to other policies provided you do not opt for outpatient cover and you take at least some excess and/or copayment.

Again - how old are you? And do you have any chronic health conditions?

Are you moving to ThalIand permanently, or just visiting, or going back and forth each year? Travel policies are much, much less expensive than regular health policies but cover only emergencies and you need to be ready and able to fly back home for treatment once stable enough.

How old?  And which conditions?

Makes a huge difference in terms of what policies, if any, you would be eligible for
