Yes, we live in Rayong and that's where our TM30s are for and where we do the 90-day reporting. But my employer is based in Bangkok so we have to go to CW every year for the Extensions. I had assumed a transfer to a new passport will also have to be done at CW, but it would be great if I didn't have to take my son out of school to do it!
Hopefully someone can give definitive advice - maybe I should start a new thread, rather than hijack this one?
Givernment eye hospitall in Nakhon Phathom

They also have a small branch in Bsngkok (no website) but I do not know if they could do such wn extensive procedure there. The main hospital in Nakjon Pathom probably better option. Bring a Thai speaker with you.

Are you sure you need this procedure? Which ophthalmologist advised it and for what condition?
Both myself and friends all had multiple (smaller) accidents here over the years (talking 5-10-15 years) while never anywhere else. I agree entirely that it has become absolute crazy here in CM due to mainly the cars driving on 2 lanes, too slow or crazy fast.
But even to take a taxi to the other side of the city here became just like being in Bangkok, in terms of the time it takes. Aside from the fact that a taxi in BKK is actually cheaper than it is here for the same distance too.
Guess you should consider to leave like I did too months ago. The burning season is also coming soon.
