The 90 day is the TM47, you do not need to provide it. You should have the TM30 though.

A completed tm47 and tm30 are required to renew 90 days

Pattaya: Last week I had to fill in a new TM47 and hand in passport copies.
The barcode on the last Slip in the Passport no longer seems to be scanned.

OP here.  Went to Immigration Jomtien today at 1:30 pm, done and out by 1:50.  TM47 was required as was an active TM30.  I always stop first at the copy shop adjacent to Immigration and they make the necessary copies: B15.

Thanks for the responses.

Here in Phitsanulok, no forms necessary, they put your passport in their reader, out comes a printed sheet, which they tear in half, give you the top, and bin the bottom.

Jomtien require completed TM47, along with copies of passport id page and latest extension stamp. No copy of TM30 needed as it's stapled to the passport

As already posted above Jomtien signage indicates a copy of TM30 is required. In my experience every single application I've made in Jomtien has required a copy of TM30

Copy of yellow book suffices for me. I've never filled out a TM30.
