I didn't see a puff of smoke. Watched a few times maybe I'm missing it. What I see is what appears to be a complete loss of thrust from both engines. One engine is enough to take off.
I don't think so. I think it was dust blown up by the engine as the aircraft rotated.
But to me there is now no doubt that the flaps were not set to 'take off' position. The aircraft stalled about 12 seconds after take off.
