Does anybody know where to get a Yellow Fever injection and a shot card in Chiangmai?

I have yellow fever. Have had a love of chinese girls for many years. Although I am the one usually giving the injections, I know nothing about cards. There is no cure wanted.

I got mine on Sri Donchai Road but don't recall the exact location as it was 4 years ago but if you start at Suriwong book shop and go away from Pantip Plaza it is on the the opposite side of the road likley walking distance. It's a health centre rather than a hospital and white in colour.

Pretty vague but maybe someone can get you closer?
