I need a urgent help.

15 days ago i broke my right hand by punching in wall my last figure is fracture and then I did plaster

in my country.

Yesterday I came bangkok to make checkup and he informed the joint not fix properly which done in my countery and itts more then 45 degree band and doctor wants do operation put some pin on it and plaster for 2 weeks.

I am not understanding what to do, pls help.The doctor I meet in burmungrad name Dr Sumroeng. PLEASE ADVICE THE BEST DOCTOR FOR THIS IN THAILAND.

If the bone is at a 45deg angle, it needs to be fixed or there will be a shortening of the bone with loss of some function. It sounds like it is the bone in the hand (metacarpal) and not the digit (finger bone) and the one of the small finger is not that critical and some shortening can be accepted but not a 45 deg angle.

At 15 days, it would be almost healed and the operation would be slightly more difficult but it is not complicated surgery and the Bumrungrad doctor should be able to do it.

If for some reason you do not want to have him do the proc edure another doctor i would recommend is Dr. Panupan who is a Professor at Siriraj and has private practice at Phyathai 2 hospital Sundays 9 - 11 Am and at Chao Phya Hospital Thirsdays 6-7 PM. he is the head of the microsurgery team at Siriraj but due to this, has only very limited time for private practice hence it would take you longer to meet him and get the procedure scheduled whereas the doctor you have already seen has daily hours and can probably do the procedure quite soon.

I have not had any first hand experience with Dr. Sumroueng but basedc on his qualifications I would feel comofrtable having him operate on me.

My X-ray saying

-FRACTURE AT DISTAL 5TH METACARPAL WITH MINIMAL ANTEROLATERAL DISPLACEMENT.

-NO NARROWING OF JOINT SPACE.

-NO ABNORMAL SOFT TISSUE CALCIFICATION.

After getting the advice of you I went immidiatly to Phyathai hospital but Doctor Panupan had left that time.

My question -

The doctor at bumrungrad Mr. Sumroeng had already did a plaster to me yesterday.

He asked me I had 2 option one is plaster other is surgery and I only advice plaster.

But now I am thinking to go for surgery as some of my friend saying there will be problem

in future.

SHOULD I DO THE SURGERY OR NOT.

None of us can tell you if you need surgery or not, we do not have access to the Xray films nor am I, at least, a hand specialist.

Your first post indicated that the doctor had said you should have surgery, but your last post seems to say he said it could be treated by either surgery or a cast (plaster) so I am a bit confused. Did the doctor specifically recommend surgery or not?

The doctor you saw is a specialist in the field with good qualifications and if he recommended surgery, you probably would do wekll to have it done. As FBN noted if the bone is at 45 degree angle surgery is advisable to prevent shortening of the bone.

To put your mind at ease, see a second specialist and specifically ask if surgery would be likely to give a better outcome than casting alone. This doctor at Samitivej is available this evening (but get your film from Bumrungrad and take it with you for him to see):
