For the annual extensions based on retirement done in Thailand, there is no requirement for either a police report or a health form. That would apply whether you originally entered on an O or an O-A.

For an extension based on retirement the qualifications are simply:

- you are over 50

and you show:

an income of 65,000 THB a month

OR

800,000 THB in a bankaccount in Thailand for at least the last 2 months (3 months for any further extension)

OR

a combination of yearly income and money in the bank, totaling 800,000 THB.

Money in the bank has to be proven with your bankbook (updated) and a letter from your bank (your bank will know). The letter from the bank has to be the day before or the same day.

Incoem from within Thailand is proven with tax records, income from abroad with a leter from your embassy.

There is no need for a police check or medical certificte.

Actualy, only when you apply for a O-A visa is the polcie and health check required. For a non-immigrant O visa based on retirement it is not.

The form can be downloaded from here: http://www.immigration.go.th/nov2004/download/tm7.doc

print it double sided, they won't accept it if you print it on 2 seperate pages.

Can't help you with the specifics of your immigration office.

At Suan Plu/Bangkok last week, they accepted my 2 separate pages TM-7 which I had downloaded.

You were lucky, many offices won't accept them that way and will require you to fill out a new form on the spot.

As I understand it Suan Phlu now uses there own computer generated form (with the digital photo/fingerprints) so the one you give them is extra; so that is probably why they were not concerned.
