What pages of the work permit must be copied and submitted for proof of address in lieu of obtaining a residence certificate for DL renewal. I assume it's only 2, 3 and 4 since 5, 6 and 7 refer to your place of employment.

I always copy them all.

Why not be safe and take 2 copies of all pages? It seems that people working for government agencies change the rules with no real pattern, so it's best to be prepared.

You can get the photocopies done for 1 baht per page when you're there. They know exactly what to photocopy.
