Yes, at Swampy after security, 200 Baht service charge on top of the fee (single and multi available).

Sign says "Re-entry Visa".

No photos or photocopies needed, takes 20 mins or so.

EDIT Make sure you use the security entrance in the centre of the building, the one at the far right does not have a desk.

At Don Mueang it's actually after the immigration desks, tell the immigration officer you need a re-entry permit. I understand there's no service charge at DMK.
