Need some help from a US Citizen who has recently obtained an affirmation and have been divorced. I am a US citizen that has been divorced. I have downloaded the one page form from the US Embassy site, http://bangkok.usembassy.gov/service/marriage.html.

In some of the posts that I have read here from other US citizens they mention two pages for the affirmation document that need to be notarized at the US Embassy prior to getting "approved certified" translations and then their visit to the MFA.

The downloadable form is just one page, do I need to get two of the same forms notarized at the embassy and then both translated and then present at MFA and then at the Amphoe?

You only need the one form.

I think there was two forms before but they have not combined the 2 into one form. Perhaps you are reading info that was posted prior to June 2015 when the form was revised.

You might need the divorce certificate if asked for it by the Amphoe.

Thanks for the response ubonjoe, I respect your answer to my question. I will ensure that I bring my divorce decree for the Amphoe, just in case. Will do our marriage in Bangrak Amphoe since they seem to be the most experienced with marriages with foreigners.

Once I complete everything I will post my experience to bring up to date for the next person.

Hi,

I am not a US citizen so I can't say what your procedure is but I can tell you that you only need one copy of the affirmation to marry certificate which is then stamped at the MFA and given over at the wedding.
