Power Of Attorney

hi all

i need a standard copy of a power of attorney in thai (without letterhead etc);

at the suggestion of samran to post here, i thought i'd ask:

its Bai Morb Amnat in thai transliteration , so says samran.

i was given a standard copy in hebrew but would like a thai version as well and since the hebrew version is also from the internet my lawyer suggested that there should be one in thai as well, somwhere. (my lawyer needs it for my boyfriend just in case we run in to visa/work permit problems with immigration here in israel, boyfriend signed in hebrew w/oral explanation but we want one in thai for full understanding of what it says)
