The new personal information form

I will be renewing my retirement visa shortly. Has anyone experienced having to fill in this new Personal Info. form at Jomtien Immigration?

You apparently are asking about applying for a one year extension of stay based upon retirement it is not a visa.

No report for Jomtien wanting it. In fact there have been only been a handle full at the most on anybody having to it.
