My personal experience from family/friends is that cancer care is mainly OPD not in-patient.

I have been searching for an Asian based insurance company for health insurance that offers good OPD cover for cancer care in Thailand

European or US companies are way too expensive.

The only decent level of OPD cancer cover I can find is with Pacific Cross but who are Pacific Cross? A corporation incorporated in Samoa?

I like their OPD policy but is it a coincidence that the only Asian insurer with good OPD cover is incorporated in Samoa?  My gut instincts are to avoid any company incorporated in a tax haven like  Samoa... but would HK or Singapore be any better?

I would prefer to deal with a company like BUPA as I am British and familiar with this company but their OPD cover is terrible.

Be aware that Pacific Cross reserves the right to increase your premium based on claims history. So if you get cancer or any other major illness involving big claims - or any chronic illness that leads them to think you will have a lot of claims in future -- they can raise your premium as high as they want, well above normal age related increase. You will find similiar language in the Thai-based policies, at least all that I have examined.

Actually most inpatient-only policies, including BUPA Thailand, do cover outpatient cancer treatments and day surgeries. (The info on their website is very incomplete, you need to get the actual member guide booklets).  It is rare for any policy not to do so. But the problem is as above.

If you stick to IPD cover only (with day hospitalization covered inc. cancer tx), the European insurers are not all that more than local ones in terms of premiums - especially if you accept a deductible (excess).

Sheryl, did you already find an insurer that suits you after reviewing all offers ? Is AIA on your list? Their Asia-Pacific coverage would suit me well but I did not look into all details yet

No, I am still looking. As I have a current (locally issued) policy, I am taking my time.

AIA is not on my short list as the plans I have seen seem geared to residents of  Singapore (special relationship/billing arrangements  with local hospitals in Singapore) and, most importantly, coverage ceases at age 75. Seeing that, I did not read further.

But it is possible they have other products I haven't discovered.
