I think you will find that doctors, (not sure about nurses), pay their debt back by working in govt hospitals for a few days a week. that is why you see so many do private practice and govt work. They don't actually pay any cash back. -

Totally depends on which office you use, mine in Sakon Nakhon is just your passport, they scan it and print out the receipt. -

From a "Thai visa expert" The letter to cancel your extension sounds great, BUT I think you're way better off cancelling your own work permit (seeing as it's YOUR booklet anyway). Go cancel the work permit, then take the letter from the company and the receipt that you canceled the work permit to the immigration office. They will cancel your current extension and stamp you in to the date on the letter. Then you've officially canceled everything the correct way and won't have any problem down the road with either the Ministry of Labour or the Thai Immigration Office.

Yes, it is charged at the same rate as overstay, (but it is not an overstay charge), and yes you will receive a receipt, therefore allowing you to apply for a new extension. -

I will copy what I answered on the other post for you here; Yes your extension may well expire whilst outside, and yes you can enter on a new DTV BUT If you do not cancel the Non-B extension before you go, expect a 20k fine when you next visit immigration for an extension. You signed form STM2 acknowledging the terms. -
