Samui Residence Certificate.

Does anyone have an up to date copy of a Samui Residence Certificate application form online? Or could point me to one?

 

I need to apply next week in order to renew my driving licences. 

 

Requirements. Am I missing anything below?

 

Copy passport photo page.

Copy visa page.

Copy Blue book.

 

Trying to get everything prepared beforehand to save time and hassle.

Do we need to fill out a form for these now? Its been about 2.5 years since I needed one, but back then I used to just go to the immigration office with my passport and photos and 500 baht and ask for them. No forms needed.

Not done one for a while, but I recall I had to fill in a form last time. 

It's not the end of the world if I don't have one. I can always get one when I arrive if needed.

To my knowledge you need a letter from Immigration confirming your address, or a Yellow House Book with your name; Blue House Book is for Thai residents only.

I had one done a few weeks ago. Went to immigration, they just wanted the Passport and 1 photo (If you have car & bike license to renew you will need two photos) and THB 500. There was no form at all. Took 10 minutes and was the easiest bureaucratic procedure I so far had in Thailand. 

 

*Disclaimer* I might just have been lucky that it was that easy. As other people say, I expected to need blue book/ rental contract with the address visible, so probably best to bring it with you.
