1. What does "certified copy" mean? Is it just a photocopy with my signature? Or can I certify it in Chiang Mai with help of thai lawyer? My embassy is in Bangkok, certifying documents using my embassy would be a pain.

2. "Document(s) stating that the applicant is used to work or reside in Thailand." Do I need this? I have only a tourist visa and letter from landlord.

3. "Letter explain the purpose of request and which country to use (only one country ) or write in the application form that can download from our website : www.pcscenter.sb.police.go.th"

Can I just write something like "The purpose is to fulfill request US Embassy for additional documents for visa application" or do I need some document from US Embassy?

1. Your signature on them.

2. Not needed if you have number 3.

3. The embassy should of given the letter to you if they require the police clearance. The consulate in Chiang Mai might be able to provide you with the letter. I am sure it is a standard letter that they put your name on.
