Depends on who you ask.

1.  Insurance companies trying to sell insurance - Very soon.

2.  Other countries trying to increase their share of the expat retirement market.  - Any minute.

3.  Thai officials trying to get a kickback from insurance companies -  July 1, 2019.

4.  Newspapers with a lot of insurance companies as advertisers - July 1, 2019.

 

The real answer?  Is a matter of opinion.  I'd say July 1, 2099.

People getting an O-X visa are already required to have it. And further required to get it from one of a few Thai companies whose policies are not a good deal to put it mildly. Not surprisingly there have few takers for this category of visa

There has been an announcement that the same will be a requirement for the O-A visa and it was supposed to start around now but does not seem to have. I expect it will though...but hopefully allowing international policies and wider range of local ones.

Nothing has been said re insurance for extensions of stay to date.

There has long been discussion of introducing something for tourists and a recent newspaper article on it but still being formulated. I think it will eventually happen and likely get tacked onto air fares, not sure what they'll do at land borders. Expect it to be very minimal (i.e. insufficient) cover and for emergencies only. But also quite cheap.
