Don't know where else to post this. Equiniti didn't pay my Service Pension this month. I called them and they said they have been sending 'Proof Of Life' forms to me since September last year but I haven't received them. Once we dug deeper, turns out they have been sending the forms to my old address! (You couldn't make it up!). I now have a copy of the form, but would any former Officer who is in receipt of a Service Pension and living in Central/South Pattaya be kind enough to witness and sign the form for me please?

 

The form says it can be "An Officer on Active or Half-Pay or Pension list of any of HM  Armed Forces". I'm assuming that could include a Warrant Officer.

 

(If you are worried about Opsec I can PM you a copy of my service discharge docs).

 

I'm too much of a cheapskate to pay a Notary/Lawyer. Any assistance would be greatly appreciated.
