Was his application for extension of stay based on an original OA - retirement Visa actually refused at CW because of not meeting the health-insurance requirement?

Or did he go for information and was told by an IO that it would be required for his extension of stay?

When actually refused, please do provide some additional detail (e.g. date of last day for permission to stay, options offered by the IO to him to regularize the matter, any other relevant detail).

Exactly. You can apply for a 1 year extension of stay 45 days early at CW. Under the current circumstances your friend should apply as early as possible. If your friend’s application is actually refused for lack of insurance then there is time to get insurance (if that is what he wants to do). Investigate insurance options now so you have a plan now. It may take at least a couple of weeks to purchase a plan and receive an insurance certificate.

Only extensions of stay for Non O-A visa holders.

What is he planning to do?

 

One of the frustrations is that verbal “advice” at CW is often unreliable. If they are handing out written instructions saying that insurance is required then that would be good evidence. Especially if a photo of the written instructions was posted. But, as a Peter Denis has asked, an actual rejection after submitting an application would be very interesting.
