You'll need only your passport and any original divorce/death certificates. You might wish to prepare the affirmation of freedom to marry before you travel. Also see the embassy's guidance on how to marry in Thailand.

I filled in the affirmation to marry and took it to uk embassy (married twice before in uk) all they wanted was decree absalute from last divorce and passport picked it back up next day, got a translation then went to local ampur answered a few questions then was hitched (again) easy me thinks
