I left thailand a few months ago with a reentry permit. (I have a O visa)

I canceled my condo contract. I will be back to thailand soon. I will go to a hotel for 2 days after my flight before to go see my girlfriend where I will stay for a few weeks.

what adress I need to write down on the thai entry form. the hotel adress, or my girlfriend adress?

There is no entry form

He means arrival card. TM6

If needed anywhere I would use the hotel.

Don't worry about it.

The TM6 is a nonsensical form that has been temporarily suspended (at airports) and some think will be not reappear on the landscape..
