You cannot cross the border to Myanmar at any of the crossings.

The nearest crossing from Chiang Mai is the crossing to Laos from Chang Khong in Chang Rai.

Yes

It depends on the origin of your passport.  I received a 45 day yesterday, 10-25-22, on a US passport.  I was with a couple and one of them was on a Romanian passport.  They required him to prove a flight out AND told him they were not supposed to issue because he had already stayed 3 months, however, he was with his Thai fiance and they allowed it.  Review the list of countries and their visa stays.  That was at Ban Laem.
