Thai Driving Licence form - where to download?

Does anyone know where to download the correct latest Thai Driving Licence form so I can print it and fill it in before going to the and Land Transport office?

There’s no such thing... 

Because that's what I've read everywhere and even been told on this forum. Why would there not be an application form to fill in?

Before you go to Land Transport, you still have to go online and take the driver's test, complete it successfully and print out the certificate.   And yes, from all I know the test is in Thai.

Why would there have to be an application form to fill in ???

 

I’ve renewed my licence a handful of times and obviously applied from new the first time - there was never a form involved. 

 

Anyway, forget the idea of filling in a form before you go there...  No form is needed at all.

 

That’s at DLT Area 3 in Bangkok, perhaps its different in other provinces with each individual office having different rules / process.

I always believed a form was being filled in for you by 'she who must be obeyed' up the steps (in Pattaya)....

I just completed the online, 1 hour video for renewing my long term license.  Both motorbike and car.   (one test does both)
I had to make an appointment.  Which I did at Phangnga because it was recommended and I'm glad I did. 
I was the only customer up there, with about 10 people bored at their jobs and eager to help me.  
They gave me all the info I needed and helped me make the appointment,, and create an account and a list of all I needed on my appointment date. 
I could've had an appointment 4 days away but waited and gave myself an extra week to get everything I needed: Medical certificate, residency permit, etc.
Now the one hour test only seemed to work on my laptop, not on my phone.  Not sure why.   
But you have to watch it as it sometimes asks you questions during the hour to make sure you're watching. 
I will go for the license in a few days.    
Hopefully I have everything in order.  
I know the drive up there sucks. but to me, it's better than waiting in long lines and have non-smiling Thai's not being so helpful at the crowded DLT in Phuket town.

At Pattaya DLT there used to be a form long ago. Now they just take your package of docs and enter the information into the computer, and once they take a digital photo they print out your license.
