There is no way to predict any increase or grandfathering. 

I am sticking with my retirement extension for the fact it is has a long record of stability, relatively low-cost / low-hassle, and possible grandfathering. 

If I were under-50 I would do the DTV over any other option.

Yes got one at SCB but I've shown my Yellow Book and DL.

3 Friends of mine have just applied for DTV at Jakarta
1) Simple one page letter of employment from company in Russia, allowing remote work and independent choice of location. In English.
with Logo, Address, Website, TaxID, Company Reg.No at the top.

2) Simple ending balance e-statement from Russian bank, not stamped, in English.

1b) one of them used the medical route, same dental clinic I went to, in Thai.

All of them have successfully received the visa slip.
