i can find the WP5 form online but they are all the old ones.

Now they are called the: "Department of employment, Ministry of labour and social welfare"

and this is stated in the new forms' head.

Have just been told by somebody at the Labour Ministry that they now use again the old forms "Ministry of Labour"

we have this form and it is available online, so no problem.

How come they keep on changing the name of their department/ministry every year?.

... don't know, but also the revenue department has changed their por ngor dtor 1 form so i can't quite rely on the "sample"-forms I paid an accountant about 2,000 Baht a month to fill out for me until I figured, I could do it all by myself in 20 minutes - (quite a good hour rate I saved for myself on that behalf). Nevermind, the changes to por ngor dtor 1 aren't but cosmetic - the fields to fill out remain the same.
