Yes, a Thai person needs to apply in Bangkok, read this http://www.vfs-uk-th.com/ and then look at the section of the forum for visas to other countries and ask there if you have any specific questions.

No Airline ticket should not be bought. Your girlfriend just has to go to a travel agent and get an itinerary in her name. Its usually free.

Also there is a Consulate in Phuket, its in Rawai.
