So I have a non o visa multiple entry for 12 months, how is it that I can get another 90 days out of my visa to make 15 months? Do I leave a little early and return, or leave as normal on the day or day before expiry?

As the OP's question has been answered may I ask what happens visa wise when within those last 90 days given one decides to go to a neighboring country for a few days then return to Thailand? Do the they simply leave the 90 day visa that is stamped in the passport or do they cancel that and give you a tourist stamp?

If you get a re-entry permit before you leave Thailand, on your return you will get a new arrival stamp (not a visa) with permission to stay until the same date as the previous arrival stamp showed.

Without a re-entry permit, you will get an arrival stamp (not a visa) with permission to stay based on a visa-exempt entry if your nationality qualifies for it. (The linked list has not yet been updated to reflect the addition of a few more European countries). Alternatively, you can buy a visa on arrival at a limited number of entry points if your nationality qualifies for it. (Also this list has not yet been updated to reflect the change that took effect earlier this month)

So even if you have a Multiple entry O visa the last 90 days given (making 15 months) does not qualify you to come and go on those last 90 days without having to obtain a "re-entry permit" from your local immigration office? I was planning to go to Laos via Friendship Bridge 10 days in of the last 90 days.

There will be an expiry date on the actual visa issued, once that has passed if you leave and re=enter by land you get 15 days by Air 30 days (dependant on Nationality).

One is a Visa the other is a permitted to stay, if leaving the day before the visa expires you get the extra 90 days permitted to stay, but if you leave within that additional 90 days period you have nothing and are back to the 15/30 days as stated above unless you obtain another Visa whilst outside Thaialnd.

A visa is only valid to be used for entry during the period indicated when issued by Consulate. The amount of time you are allowed to stay in Thailand is your permitted to stay and is what immigration provides when you enter. They are separate matters. The only way to keep your current permitted to stay alive for an exit/return is by use of a re-entry permit.

During the period the multi entry visa was valid you were not concerned with keeping the permitted to stay as any new entry got you a new permitted to stay. That is not the case after visa validity expires.
