Is this necessary to complete for a Non-B Education Visa?

My school gave it to me, but I've never heard of it before, and my last school (different) didn't bother with it, and I didn't have a problem with my visa

I couldn't even find much information about it on google, except a few mentions from 2006 and 2007

Some immigration offices have been reported as asking for it as part of proof of address needed for extensions of stay (not a visa).

There was a recent report that CW wanted one retirement extension but let them get it without TM28 but told them they would need it next year.

If you are using CW you might want to have landlord sign it or you might have to make another trip if they want it.
