What do you mean "blood pressure 45 to 58". Do you perhaps mean your heart rate is 45 - 58? (Not the same thing as blood pressure, which has a top and bottom number).

How old are you? On any medications?

Sorry there must be some confusion a B/P of 45/58 is impossible !

You might want to choose a hospital that has a heart center if you want to have a test specifically about your heart. All hospitals can make a simple ECG but only some have a specialist that can interpret it.

Also not all hospitals can do a stress test (EKG under stress - running on a belt) if that would be necessary.

If you just want a general health check you can buy a package in some of the private hospitals. That way you're usually much cheaper of.

PS. He probably means a diastolic between 48 to 58 mmHg?

There may be no connection between the OP's feeling tired and his blood pressure (whatever that may actually be). Best to get a full checkup to evaluate overall state of health.

I think edmatty is friend of a friend of mine who asked me for some advice about his situation and I suggested he contact Sheryl and the other helpful people on TV on this thread.

He thought I would be a good source of information as I had recently been involved with the medical profession here in Thailand and subsequently in India, in relation to my heart and hip.

Like me, he has little trust in some/all the private hospitals here in LOS to give an unbiased diagnosis that is based on medical need and not mainly profit for the hospital.

With the help of people on this forum, I managed to come up with a good solution - which in my case involved a flight to India to have my hip operation and not having to do anything more to sort out my minor heart arrhythmia.

His post is very short and does not explain any of this, as he is new to this type of forum.

So perhaps I could add what I think is some of what he is looking for.

I think he would like some advice as to where he could go in Thailand to have a check-up to see why he is so tired all the time.

He would like the name of a hospital that he can trust to give him good advice and not just run lots of expensive tests for the sake of it. I think we all know the types of hospitals that do this to us!

I cannot speak for edmatty as I don't really know him, but like AYG says, his comments about his heart may not be that helpful. He just feels tired and needs a full check-up to find out why.

By the way, he was considering a full MRI scan, which following Sheryl's advice in a previous thread via our mutual friend, I talked him out of. However, you can see he is 'lost' in this world of private health care and really could do with some help!!!

The name of a hospital that would give him this full unbiased health check-up would be a great start.

Thanks for helping him out

Do you know whether the OP is now in Thailand? And if so, where's he living?

No matter where he goes, he needs to be an informed consumer and to know what check-up tests to get. While there are hospitals that will not automatically push unnecessary tests, none of them adequately advise on which tests are needed and the "packages" are often lacking one or two things an individual may need while simultaneously having some unnecessary things. They are not all that well designed plus they are based on health problems common to Asians, not Westerners (hence the common inclusion of abdominal ultrasound, which you would rarely see done as part of a check up package in the West - it is because of the unusually high prevalence of primary liver cancer among Thais, which is very rare in the West).

In addition, if it is possible to narrow the likely problem down a bit, he might do better to consult a specific type of specialist rather than go through a check-up department (but only if it can be narrowed down).

Hence the importance of getting to the bottom of this "blood pressure 45 to 58" business. Obviously that is not a BP, but it might be a heart rate and if so and if he is neither engaged in heavy athletics nor on beta blockers, then he may have an abnormality of the heart rhythm with a resulting effect on cardiac output. So to advise him properly I do need to know this.

Also age is very significant in terms of which tests to get.

And certain medications can cause fatigue as a side effect (and would also tell me something about his prior medical history).

Happy to advise, but really need more info to do so properly. Also, please confirm he is in Bangkok area or if not, where exactly he is.

Thanks for the feedback, good advice as always.

I don't know edmatty's details, as he is the friend of a friend. My friend told me that the op was going to ask for advice on TV today, so I kept an eye out for his post. When I saw how brief the question was, I thought some further explanation may be helpful. I suspect the op will read the replies and start to become more 'comfortable' with discussing his details in an open forum like this.

I will mention to our common friend that he could advise edmatty, as a start, to understand what his 'blood pressure' and heart rate readings actually mean.

You should get a full cardiac evaluation undertaken by a cardiologist .

The slow heart rate and low blood pressure would suggest you would benefit from treatment .

Yes, sounds like you have some sort of arrythmia (abnormal heart rhythm). Which may or may not be related to the valve problem. And which may well be the cause of the fatigue.

Assuming you are in Bangkok I suggest you consult Dr.TAWORN SUITHICHAIYAKUL at Bangkok Christian Hospital. Saturdays 10 - 12 but get there early to register and be in the queue as it is first come, first serve and he is in great demand. He is a Professor at a leading medical school, trained in the US (UCLA) and is very good. Bring all available medical records with you.
