Just rock up at the tarmac, look for the Bangkok Airways ATR 72 with the cardboard sign on the windscreen which says 'Samui'. Walk up the stairs and take a seat (first come, first served). When the plane is full, they will taxi for take off and an attendant will walk up the aisle and collect payment from you.
