I am on a 1 year extension and work permit valid until June, 2015. My wife and Kid are on Non O 1 year extension as my dependents. My wife has lost her Departure Card with Stamp on it and due to some Emergency back home she will be Flying out on Sunday. Will she have any troubles in Exiting on a Departure Card with No Stamp of Arrival Date and would she be able to get a Multiple Re-Entry Stamp Ofcourse paying the Fee? If yes what documents should she have?

Ive seen it many times... People leaving at immigration have lost thier departure cards, the immigration just make them fill out another one and they go on thier way..

It's not a problem..

There is no fine or fee. Just fill a blank card at departure and apologize to the officer for having lost the original.

She can ask for a new arrival/departure card when she checks in for her flight and then fill it out and give to it to immigration.

If leaving from Suv she will not need anything other than her passport to get a multiple re-entry permit. They will do everything at the re-entry permit desk. A 200 baht fee for them doing the photo, form and copies will added on to the 3800 baht fee for the re-entry permit.

A friend just did this at CNX, and he had a copy of visa/stamp/photo page and two pictures and he just paid the 3800 for multiple. He said it was surprisingly easy.
