I need to get a Thai passport for my son, I know the process but I am in Cambodia and he and his mother are in Thailand. I know that she can apply without me if I get a letter of parental consent notarized and sent to her.

I have 2 questions: Does the letter have to follow a certain format/template? and can I get in notarized anywhere or does it need to be done at a Thai embassy or consulate?
