Confused about Thai name certificate requirement for Visa for my wife for USA. Shortly after we were married here in Thailand she changed her ID and Passport to reflect my last name and that was a little over 6 years ago. 2 questions below.

 

1. Is the embassy looking for a name change certificate reflecting her maiden name to her married name from when we got married?

I do have all of the paperwork that came with our marriage license but it is all in Thai.

 

2. Could it be in that stuff?

If so, I guess I will have to get it all translated.

Is this for a U.S tourist visa?  I don't have the form handy, but if I remember correctly, the blank on the form just asks for "former" names, or "other" names, something like that.  If so, then I don't think they need the official Thai name change certificate.

No, this is not for tourist Visa. It is for an Immigration Visa for my wife to go to the USA permanently. We got married and then she changed her surname to mine on her ID and Passport already.  I'm trying to find this document and not sure what it looks like. This is in the Packet 3 instructions.

 

NAME CHANGE CERTIFICATE(S): Required for all name changes if the petitioner or applicant (and applicant’s parents or children in stepparent-stepchildren cases) have ever changed their first name or last name. A Certificate of Same Person issued by the local district office (Thai Amphur) listing all names since birth may be submitted in lieu of individual name change certificates.

 

In our marriage registration form we received, it has this statement and I'm wondering if it enough. "Use of Surname: the female party to be married wishes to use the surname of the male party."

I'd suggest just going down to the amphur where your wife did the name change and asking them for a copy of the paperwork, the request and the approval.

 

My Thai wife's daughter did this, changed her last name.  I needed a copy of this paperwork for my U.S. medical plan, no problem, they dug it out and gave me copies.
