Bit difficult to start to pee than when I have still feel full like I have not actually done it, added to that quite a bit of aching on and off, testicles not swollen. Had it for 12 days and taken doxycyline for a week but does not seem to have done anything. No fever or blood and had prostate looked at a while back so not that. Only been to the local clinic, is it worth a hospital visit for tests?

Anti bionics should be taken for at least a month to have any effect,

other than  kidney stones comes to mind, in any case, it's defiantly

warrant a visit to a urologist for urine and blood tests and CT scan

if need be....

Yup, first thing that sprung to mind...... stones, but right, visit the Doc and get that checked out.   My mate here is from Washington State, not in the habit of drinking lots of water.   Eventually landed on his back in surgery here, shocked at how many stones the Doc pulled out, and had to walk around with a piss bag for a week or two after.  

 

I drink 2 to 3 1.5 liter bottles of water per day out of sheer habit.   He still won't listen, drinks beer everyday, hardly any water.

Disagree with that statement...........look up any particular antibiotic and the dosage required and you will see that many work within a few days and 7 to 15 days "could be considered a norm".

 

However in some cases where the penetration of antibiotics is poor, then longer periods are sometimes prescribed.

If it was urinary infection, than a short does perhaps,  if it was prostatitis, then, yes a month long dose, 

 

of course the OP,  is just posting very vague  picture,  so  whom knows........   maybe add more  history, age, whatever IMHO
