Dear All,

 

I am having problem since 1 year.  Diarrhoea, Dysentery, then constipation, then again loose stool, rectal pain.   I can not sleep in night. Sleep does not come. Sleeping problem. Too much acidity, gas, weakness, dry mouth, sudden urge to urinate.  This is cyclicly going on with me last 1 year. My body weight reduced from 76 kg to 68 kg. My Quality of life become worst.

 

I have consulted with Medicine doctor, Gastroenterologists in My country - Bangladesh and also in India.  Some suggested about IBS, some did not identify the root cause.  I have done Endoscopy, colonoscopy, proctoscopy. Endoscopy found- Antral Gastritis, Duodinitis. Colonoscopy found nothing. Proctoscopy found large Hamorroid requires operation suggested by one doctor and another doctor said operation does not require now.

I tried many medicines, nothing worked properly.


I am in great problem. My work and my life, everything become hell.

 

I kindly requesting to this forum members, Please advice me Best Gastroenterologists and Internal Medicine doctors, And also Colorectal Surgeon for me. 

 

I would like to visit Thailand in upcoming next month.  I need advice.  I heard bad things from my father regarding Bumrungrad. As he did not find it good.  
So I need real and expert opinion.

 

I am waiting for you guys. Please help a Human. I want to live, I want to live with my family.

whats your medical history, did you have any health problems before this started ? or did this start randomly ? do you have a history of taking antibiotics ? whats your diet like ? do you eat healthy ?

 

the doctor that thinks the hemorrhoid should be removed could be right, if its large then it makes sense it could be causing your rectal pain. 

 

if you have a history of taking antibiotics, that can cause stomach/gut problems as you've killed a lot of your good bacteria, take a good quality probiotic supplement and eat fermented foods like Kimchi, kefir, Miso, Sauerkraut etc.. 

 

try to eat more of a vegetarian diet with prebitotic foods like vegetables, fruit, nuts, wholegrains, beans etc ..little bit of meat and eggs, try to avoid dairy and drink soy milk or another dairy free milk like coconut or rice milk etc..

 

but see a doctor first and see what they think.

Bring all records with you, especially the CDs from the endoscopies.

 

I think since you have significant weight loss it would be wise to have an abdominal CT (upper and lower abdomen) and this will be much cheaper in Bangladesh than in Thailand so I suggest you get it there and bring the results (actual films/CD not just radiologist report) with you. This is in case there is a tumor located somewhere in the abdomen that was not reached by the endoscopies.

 

You might also get the following  blood tests before you come and bring the results with you, as it will save time:

  • CBC (complete blood count)
  • serum amylase
  • serum lipase
  • CEA
  • CA19-9

 

"dry mouth, sudden urge to urinate" are not related to your GI issues but might well be side effects of medication. What medications are you taking?

Many thanks for your kind reply. I was trying to contact with you, as I have not found any email of you. I really need your help as you are expert.

 

Abdominal CT in Bangladesh could be cheaper, but problem is in Bangladesh, There is misinterpretation of CT Scan or any test happens. Wrong diagnosis is common problem here in Bangladesh. We have good doctors, but diagnosis sometimes wrong by tests.  Already two big hospital got huge fined by government, they used expired reagents which are used in blood testing. If expired chemical used to test blood, then how we general people can trust these results value.  

 

I am very much tired, dissapointed and fed up about my health issue.  

My Symptoms are many

- Sleep does not comes naturally, sometimes require sleeping medication. I tried Nortryptiline, Clonazepam etc.

- Stomach sounds, growling sounds

- Excess gas formation in stomach and also excess fart

- Feeling weak

- my body weight was actually 82 kg. I reduced to 74.5 kg by Intermittent fasting diet + Gym.  then Oneday I become very sick suddenly.

I had Gout attack on right leg toe. then Doctor gave me Colchcine to reduce. after taking this medication, loose motion started.  I stopped it.  Then Doctor gave me Naproxen, It gave me sever stomach pain, inflammation and then loose motion. I stopped that medicine.

Then after few days later, again loose motion, I took Antibiotics Ciprofloxacin, Metronidazole.

Then after 10 days again loose motion, I took Antibiotic Azitrhomycin.

Then after every 15 days or 10 days diarrhoea or loose stool with mucus or sometimes become constipated.

I had also right knee pain. I took treatment in India, France, Bangladesh, nobody identify the root cause of my right knee pain. Some said sports injury, some said uric acid attack etc.  now this pain subsides by physiotherapy and medication.

 

But this stomatch problem persists. due to this from 74.5 kg, My body weight gradually reduced to 68 to 69.6 kg.  Body become weak.

 

- Undigested food parts in Stool

- After stool passing, itchy or dull pain in rectum area.

- Digestion is I think main problem.

- I have Fatty Liver problem. Stage II or III fatty liver. But no fibrosis. I have done Fibroscan, Ultrasound.

 

The blood tests you have mentioned, I want to do them in Thailand. I do not trust Bangladesh report.

 

I am taking Probiotics and Meberine HCL tablet for IBS.  

 

I want to come to Thailand to identify, Am I really IBS patient? or I have any other health problem, which is misdiagnosed in Bangladesh?  So, I need very experienced English Speaking Good Gastroenterologist.


Also I need to cross check with a experienced Good Colorectal Surgeon who can tell me whether I need operation now or I can do operation later...  In our country there is a good colorectal surgeon, he is the best in South Asia. but he is too much commercial, He just need money. He has reputation and good hand. But Patient service is worst and not clean in his hospital.

 

Please advice me Is this Christian Hospital is better than Bangkok Hospital?   Money is not problem. I want the best and experienced service from Thailand. I suffering a lot. It seems It is better to die, rather than this suffering. My quality of Life is fully hampered.

I have fatty liver problem and uric acid problem before. I follow healthy diet. I do eat Probiotics, vegetables, fruits, wholegrains.  I was attacked by Gout two times in April 2017. In May I had sudden right knee swelling and knee pain. that time doctor gave me Colchicine, it cause me loose motion.

Then Doctor gave me Naproxen which also cause loose motion. Then doctor gave me Antibiotics to stop it.

Diarrhoea stopped. It comes every 15 days or 10 days in loose stool with mucus, stomach growling sound, excessive gas, excess farting, after diarrhoea stops, feels rectal pain, itchy,  sudden urge to urination, bedtime urination during sleep.  Also sleep problem, insomnia.  Feeling weak, body weight reduced, my face and body looks shrink, my quality of life changed.  Sometimes stool 2 times, someday when it flares up 5-7 times on a day. I become very weak on that day. Some times become constipated!

 

In the morning I eat egg whites and 1 thin bread called roti.

In the Mid Moring 1 banana. Lunch I take vegetable curry or fish curry with 2 pc of roti (thin bread) made with flour.   Doctor told me, do not eat Bread made with Wheat, avoid Milk, Lentils, spinach, gas forming vegetables.

 

In the night, I eat 2 cup of white rice, with vegetable or fish or chicken curry. sometimes yogurt in the night.

Ispaghula husk with 1 glass of water. I drink 2.5 to 3 litre water per day.  30 minutes walking in the evening, I try.

No hospital is better than another, it depends on the specific doctor.

 

The best colorectal doctor is prof Chucheep as I have recommended.  he can be seen at either Bangkok Christian Hospital or Bumrungrad hospital.

 

For GI problem, I have also made a recommendation, please see my post above.

There is an excellent colorectal surgeon at Bangkok Phuket Hospital, Professor Art Hiranyakas, who has been trained overseas and is a leader in his field, and is highly respected...……….he has also helped a TV member here.

 

He runs that unit and lectures around the country and I can highly recommend him, having used him twice.

This hospital is not in Bangkok right? It's in Phuket?  Does he also consult in Bangkok also in any hospital?

Yes it is in Phuket, but does consult in Bangkok I believe.

 

If you get on to the Bkk Phuket Hospital website you can find him there and ask where/if he consults in Bangkok.
